Arrival Time
- Valid insurance card, child's immunization records, guardian's photo ID, and current medications list.
We accept most major insurance plans. Please confirm your coverage with our staff when booking. Copays are due at the time of service.
Please arrive 15 minutes prior to your scheduled appointment time to complete any remaining paperwork and register in our system.

You can schedule by calling our local Elizabethtown office directly. Our staff will help you set up an account and find a time that works for your family.
